You can file a grievance in person or by mail, fax, or email. If you need help filing a grievance, Pat Gillett, SSD-Civil Rights Coordinator, will assist you.
You can also file a complaint with the Missouri Department of Social Services by filling out the Client/Applicant Complaint of Discrimination form posted online on the DSS website https://dss.mo.gov. The form can then by emailed to HRC.OCR@dss.mo.gov or mailed to Missouri DSS Office of Civil Rights at P.O. Box 1527, Jefferson City, MO 65102-1527 or by contacting by telephone at (800) 776-8014.
